Children’s Case Management
Service Coordination for People with Developmental Disabilities is here to help you plan for the needs of your child and family, and to connect you to developmental services and supports in your community.
We provide this support for families whose children have been diagnosed with a developmental disability or autism spectrum disorder.
Once your child’s eligibility is confirmed, your case manager will complete an Initial Needs Assessment with you. Your case manager will assist you in linking you to community resources, completing a person-centred plan and provide ongoing support, as required.
Case management services may also include assistance with the completion of applications for community supports and services. While some community resources can be accessed by contacting them directly, some programs in the developmental services system require a referral from Service Coordination.
