Intake for Case Management
Service Coordination provides information, support, and guidance to children who are diagnosed with a developmental disability and/or Pervasive developmental disorders (Autism spectrum).

When you first contact Service Coordination, you will be connected to a Assessor/Service Navigator. He or she will assist you with the registration process.
To be eligible for services in the adult developmental services system, you must have been assessed by Developmental Services Ontario Eastern Region (DSOER) and referred to Service Coordination for People with Developmental Disabilities’ Case Management Services.

Please note that Case Managers rotate on a weekly basis to fill the intake role at SCS and we suggest to contact the reception to reach the current Intake Worker.
